Goodness Team

If you have been a provider in healthcare for nearly any time at all you’ve probably seen the gaps that exist around care transitions. We saw that ourselves first hand as we were rounding providers in SNFs. When the pandemic aided the removal of Telehealth restrictions we were able to pivot our practice toward an area of greater need and greater contribution: transitional care. We scoured all the best research about reducing unplanned re-hospitalizations (over 86 published studies!) and found one common theme throughout:

Increased clinical contact = less unplanned re-hospitalizations.

In fact we’ve found that frequent, low-barrier check-ins with patients can prevent over 60% of unplanned re-hospitalizations. We all know that healthcare has its share of problems and while we can’t fix all of them, this is an area where we, and you, can make a real difference.
